DexCom Store. New orders are handled through Sales Support using the steps

1. Getting started
DexCom can help guide you through the insurance and authorization process. Each
insurance company and unique plan has their own set of requirements to approve your
DexCom SEVEN PLUS.
DexCom has a team of experts who will help guide you through this process to help
you get your DexCom SEVEN PLUS approved by your insurance company.
2. Complete these forms
In order to start the process, please complete the Getting Started Forms below:
Once you have completed these forms, please fax them, along with a copy of your
insurance card (front and back), to DexCom's Sales Support Team. Note that one of
the forms requires a doctor's signature.

DexCom will begin a benefits investigation with your insurance and then let you
know your plan's specific coverage for the DexCom SEVEN PLUS.

Although many insurance plans are now covering the DexCom SEVEN, some do not cover
it at this time. Call DexCom to learn about the current cash programs that may be
available for you.
To purchase a SEVEN PLUS without using insurance, you will still need a prescription
signed by your physician.
