This Free Trial is valid for one CGM sensor. The Free Trial voucher must be presented to your pharmacist along with a valid prescription. This Program is limited to one CGM sensor per patient.  If your Healthcare Provider has already sent a prescription to your pharmacy, you will only need to present the voucher information.
