DEXCOM G7 SUBSCRIPTION

Subscription

• You may choose a monthly or quarterly subscription to purchase your DexCom G7 products from the DexCom Store.
• The prices of G7 products are indicated on the DexCom Store. The total price of your subscription ("subscription fees) depends on the number of G7 products ordered, the term of your subscription (monthly or quarterly) and the shipping method selected. Subscriptions provide you the discounts that are shown on the DexCom Store. The subscription fees for your subscribed G7 products are shown at checkout when placing your order.
• Unless you cancel the subscription through your account in the DexCom Store, the subscription fees will be automatically charged and payments processed on your selected term (monthly or quarterly, depending on the type of your subscription) from your payment card listed on your account in the DexCom Store.

Payment

• By accepting these Subscription Terms and Conditions, you are authorising DexCom to charge your payment card for your subscription fees. You are also authorizing DexCom to use a third party to process your payments.
• Subscription fees will be charged to your payment card before the shipment of your G7 products. Any amount that is payable on a day that is not a business day will be charged on the next following business day. Unless otherwise specified in the DexCom Store, amounts are denominated in British Pounds.
• If DexCom is unable to deduct the full subscription fees from your payment card as described above, DexCom will attempt another deduction after 72 hours. If settlement is still not possible after 72 hours, DexCom will wait 14 days for you to provide a new payment method. If a successful settlement is still not possible after 14 days, DexCom will cancel your subscription and inform you by email.

Delivery and Title

• You can choose your shipment dates in your account in the DexCom Store. The subscription fee will be adjusted accordingly and deducted before the changed shipment date.
• G7 products will be shipped to your shipping address that you provide in your account in the DexCom Store. It is your responsibility to ensure your shipping address details is accurate at all times. If you fail to notify Dexcom of a change to your shipping address after products are sent to you, you will be responsible for the collection of that delivery at the shipping address you provided or the cost of any replacement products.
• Title to the G7 products you purchase will pass to you upon shipment from Dexcom. Where applicable, separate charges for shipping and handling will be shown on the invoice(s).

Cancel Anytime Policy

• You can cancel your subscription at any time. Your subscription will promptly expire after you cancel the subscription on your account in the DexCom Store. Cancellation must be made at least two days before the shipment of your G7 products. Within two days after cancellation, we will not charge further subscription fees or make any further shipments of the G7 until you make a new subscription.
• If you cancel your subscription after two days prior to the shipment of your G7 products, returns and refunds are subject to DexCom’s Return Policy.
• If you wish to change your subscription,

• you must cancel your previous subscription first. You may subscribe for different options thereafter on your account in the DexCom Store. The new subscription will become applicable to your subscription fees deducted from your payment card and deliveries of your G7 products.

Use and Maintenance

• You must use G7 products with caution and follow any guidance from your health care provider to understand how to properly use your G7 products. You must use G7 products in the manner set out in the instructions for use. You must not, and you must not permit anyone else to, modify, disassemble or perform repairs on your G7 products. Please refer to the Terms of Use.
• If your G7 products purchased in the DexCom Store fail to operate during the relevant term of operation (for example, up to 10 days for sensors), you must promptly notify DexCom of the failure. If DexCom determines that the G7 products were defective and covered by the applicable warranty, a replacement will be sent to you at no charge. G7 products damaged and not covered by the applicable warranty will not be replaced free of charge.

Termination

• You may terminate your subscription at any time. Please refer to the Cancel Any Time Policy above.
• If you fail to make any payment as described in the Payment section above, Dexcom may suspend any support or other services to you relating to your G7 products and may, by notice, terminate the agreement with you regarding your subscription to G7 products.

Limitations

• DexCom does not provide medical advice or any medical examination. It is your sole responsibility to seek medical advice before using G7 products.
• To the extent permitted by applicable laws, DexCom expressly disclaims any further representations, warranties, conditions or other terms, express or implied, by statute. Please refer to the Warranty set forth in the instructions for use.

Final Provisions

• These G7 Subscription Terms and Conditions will be interpreted in accordance with the laws of England and Wales. Each party hereby irrevocably submits to the exclusive jurisdiction of the Courts of England.
• You confirm that you have been provided with this information in a clear and comprehensible manner and that by checking the box “I HEREBY CONSENT TO THE G7 SUBSCRIPTION TERMS AND CONDITIONS,” you also agree that DexCom can charge the applicable subscription fees to your payment card that you provided in your account on a recurring basis on the terms set forth above and described in your account.
